Please Bring a valid Photo ID such as a passport or driver’s license, Also your Vaccination or immunization record.
We complete the I-693 form after conducting the medical exam and reviewing the TB screening results and other required tests per CDC guidelines. We also carry all the necessary vaccines for all age groups as per Storage & Handling recommendations and guidelines by VFC.
